The Robin Hood Health Foundation needs a new NHS Link Worker & Wellbeing Advisor to join the Hera team
The role involves working in partnership with patients in primary care, to identify what matters to them and to help them access the community resources they need to be as well as they can be.
This work is known as ‘social prescribing’ and helps to deliver the new NHS Personalised Care strategy
The Hera Project has active links with voluntary and public sector partners in our community, and strong creative input from our renowned team of artistic practitioners, who provide a wide range of group activities shown to improve health & wellbeing.
The role is 0.8 FTE, and currently equivalent to band 5 on the NHS pay scale
